Proxy Votes
Please read carefully before assigning your proxy vote.
1. Only members in good standing who have made a financial contribution of record
during the current or previous fiscal year and preceding the date of first notice of the
annual meeting may vote in ASUUC elections.
2. ASUUC Bylaws provide any voting member the option to cast his or her ballot at any
membership meeting either in person or by proxy.
3. Proxy voting must follow these rules:
A. If you wish to vote by proxy, you must use this official form. The proxy form must
be dated and signed to be valid.
A proxy form may be a photocopy or facsimile of the official proxy form, but must
have an original signature in ink in order to be official.
In the blanks in the proxy statement, print clearly in ink your name and the name of
your proxy designee and the date of the meeting.
B. The designated proxy holder must be a voting member of ASUUC.
C. The proxy holder must present the proxy form for certification during voting check-
in. The proxy may be exercised only by the person or persons named.

This is to certify that I, _____________________________, the undersigned voting
member of All Souls Unitarian Universalist Church, have designated
____________________________ as my personal representative to cast all votes and
express all approvals or disapprovals that I may be entitled to cast or express at the
annual meeting of All Souls Unitarian Universalist Church on _______(date of meeting),
and in any lawfully adjourned meetings thereof, and for all purposes provided in the
Constitution and Bylaws of All Souls Unitarian Universalist Church and the Laws of the
State of Missouri.
In no event shall this proxy be valid for a period longer than seven days after the meeting
for which it is given. This proxy shall be revocable at any time at the request of the
undersigned voting member who gave this proxy.
