WWT to conduct supervision should present every his/her visit to the project with this format. If based on
the visit result there are issues, which needs to be known by the WWT Members and solutions to be given by
the board on problems observed during the visit, the supervisor should present the report to the board
members.
2. This visit report should be filed to the file opened in the name of the project.
