Anticipatory Guidance for the 4 Month Well Child Physician Visit

At this visit you can expect:
ο
Your baby will be weighed and his or her length and head circumference will be measured.
ο
Your baby will be undressed for a full physical exam.
ο
Your baby’s vision and hearing will be checked.
ο
Your baby’s development will be checked.
ο
Immunizations: The deadline for these immunizations is 5 months of age, unless a child is on a catch-up
schedule then follow your provider’s recommendations. Please provide Home Visitor with copies of com
pleted immunizations and/or catch-up schedule. Ask your provider about these:
Hepatitis B-#2 (due at 3 months old up to 5 months old)
Diphtheria, Tetanus and Pertussis (DTaP)-#2 (due at 3 months old up to 5 months old)
Inactive Polio-#2 (due at 3 months old up to 5 months old)
Rotavirus-#2 (due at 3 months old up to 5 months old)
Haemophilus influenza Type b (Hib)-#2 (due from 3 months old up to 12 months old)
Pneumococcal-#2 (due from 3 months old up to 12 months old)
You might want to discuss with your provider:
ο
Any illnesses your baby has experienced, any visits to another provider and any emergency room visits.
ο
Observations you have made about your baby’s vision, hearing and development.
ο
Your baby’s changing sleep habits and sleep routine.
ο
Your baby’s personality and how you can know what your baby is trying to tell you.
ο
How feeding is going. How to know when your baby is developmentally ready for additional foods.
ο
Ask your provider to check your child’s mouth for any cuts, sores, white spots, blisters of swelling of the
gums.
ο
How your family has adjusted to the new baby. Your work or school.
ο
