Placement Attendance Record Sheet

The chart below should be filled-in on a weekly basis by the student and signed by the placement supervisor.

At the end of the placement period, the student will hand-in this completed record to their course instructor.

Please include any additions or changes to the schedule of placements on a separate, attached sheet.

Attendance: Students, please be aware that failure to complete the placement hours that are required in order to fulfill the CSL component of your course can

result in a failing or incomplete grade for the course. An inability to attend your placement should always be communicated with your placement supervisor prior

to the date and/or time of the placement and, in cooperation with the placement supervisor, effort should be made by the student to make up the hours missed.

Verification of the reason for absence (e.g. doctor’s note) may be required in some instances. Placements should start promptly, at the beginning of the semester,

and you should attend your placement regularly, throughout the semester, as per your agreement with the community partner (other side of this sheet).

Misconduct: Any misconduct (e.g. falsifying attendance records) during the course of your placement that compromises the valuable relationship between Laurier

and our community partners may result in disciplinary measures from the University. This may include being assigned a failing grade for the course.
