Wag The Dog Daycare Policies

(Please Read and Initial)

______ 1. Day Care: All dogs must complete a temperament evaluation. Wag The Dog Daycare reserves the right to

refuse or to reject any dog that might be a safety hazard to its self or other dogs in our care or staff member. This

includes, but is not limited to dogs who are aggressive, anti-social, toy or people possessive, anxious, or

physically ill. All dogs must be at least 6 months or older to attend day care/boarding and all dogs must be

spayed or neutered. These 2 items may be waived on a case-by-case basis.

______ 2. Vaccinations: All dogs must have current vaccinations against Rabies, Bordetella and DHLPP. Please note:

getting a Bordetella vaccination is not a guarantee your dog won’t get Bordetella. Your dog will not be

admitted into Wag The Dog Daycare until we have proof of vaccinations. Three notices will be given prior to

shots being due. The

third reminder (due date)

will also be notice that you can no longer bring your dog in

without proof of updated vaccinations. Also NOTE: If you have not been here for several weeks or months and

your dog is past due for any vaccination, you will not be able to drop him off without proof of vaccination.

______ 3. Flea Protection: All dogs are required to have flea protection. Your dog must be on a veterinarian-approved

form of flea control. Acceptable flea treatments are (but not limited to) Frontline, Frontline Plus, Advantage, K-9

Advantage, and Revolution. Any dog that has fleas will not be allowed in our facility.

______ 4. Collars and Leashes: All dogs must be on leash when entering and exiting Wag The Dog Daycare. All dogs must

also have a QUICK RELEASE collar. NO BUCKLE COLLARS. THIS IS FOR SAFTEY REASONS!!!

______ 5. Toys and Beds: You may not bring your own toys or beds from home. This could cause possessive behavior.

______ 6. Food: If you want your dog to be fed in daycare, please bring your own food from home. If your dog is

BOARDING you can bring your own food or we can feed them our food.

______ 7. Service Fees: All fees for services are due when services are rendered. Acceptable forms of payment are

checks, credit cards & cash.

______ 8. Late Fees: There will be a $10 late fee for dogs picked up after closing, between 8:01 and 8:15. At 8:16 your dog

will have to stay overnight and will be charged overnight and any other appropriate fees.

HOLD HARMLESS / RELEASE AGREEMENT

Wag The Dog Daycare, its owner, employee(s), representative(s) or any other person(s) affiliated with the company shall

hereinafter be referred to as “Wag The Dog Daycare.”

By signing this form, you or your representative(s) ___________________________ (print dog parent’s name) shall hereinafter be

referred to as “Parent,” and agree not to hold “Wag The Dog Daycare” liable or sue for any injuries and/or death to your

dog(s) ___________________________ (print dog’s name) while in the care of “Wag The Dog Daycare.”

is fully vaccinated in accordance with Wag The Dog Daycare policy.

By signing this form, you acknowledge that you understand and accept the terms and conditions set forth by this

agreement.
