you to recover possession of the premises, recover rent and damages, and to declare the rental or
lease agreement forfeited.
This notice also serves to notify you that landlord/agent elects to and does declare a forfeiture of
the lease or rental agreement if the total sum of rent is not paid in full within the three (3) days.
