YOUTH BAPTISM PERMISSON FORM

Dear Parents or Guardian,

This is a baptism permission form. Your child has been counseled about their salvation and has

believed on the Lord Jesus Christ as exampled in Acts 8:36-37. Now in accordance with Acts

2:38 they wish to be baptized in Jesus Name. If you would give your consent that your child may

be baptized by immersion, please sign below.

Baptism is simply a picture of what Jesus Christ has done for us. He died, he was buried and HE

rose from the dead. Baptism means that someone is identifying him or herself with Christ and is

taking a stand for Him. This is in obedience to His Word (Bible).

One can be baptized at the end of any of our services or you may schedule a personal time. We

ask that you take this time to invite family and friends to join you in this special time.

Please fill out the following areas, where indicated, and submit the completed form to one of the

pastors or the church office before being baptized.
