5LETTER OF RECOMMENDATION GUIDELINES
We would be pleased to write a letter of recommendation for you, but we need your help to complete the
process. Before any letter will be written, you must meet with them and provide the following items:
1. Request Letter – please write a short letter requesting the recommendation.
2. Student Questionnaire – complete this form with as much detailed information as possible. You don’t
have to answer every question but the more information we have, the more complete our letter will be.
3. Application forms - Many high schools have specific forms for the counselor to complete or they may
accept the Common Application. Print any necessary forms, complete any student sections if required,
and provide these to your counselor/teacher.
Please allow at least two weeks lead time prior to the deadline for this process to be completed.
