SECTION FIVE. Only the following forms of payment will be accepted in payment of COD amounts:
1.
Cash, up to a maximum of $250.00;
Bank cashier’s check;

Bank certified check;
4.
Money order or
5.
Personal check of the consignee when so authorized in writing or by endorsement on the Bill of
Lading by the consignor.
All checks and money orders shall be made out to the consignor. The carrier will accept checks and money
orders only as the agent of the consignor and the carrier’s responsibility is limited to the exercise of due
care and diligence in forwarding such checks and money orders to the consignor.
SECTION SIX. The charges for collecting and remitting the amount of each COD bill to be collected on
shipments consigned COD .0574 times the COD amount. $45.00 minimum, $500.00 maximum.
SECTION SEVEN. Carrier will, upon written authorization from the consignor, change the status of COD shipment
by increasing, reducing or canceling the amount of the COD, subject to the following provisions:
1. The request must be received by the delivery carrier in time to accomplish the change
requested prior to effecting delivery of the shipment.
2. A charge of $26.25 per shipment will be made for increasing, reducing or canceling the COD
amount. Such charge will be in addition to the COD collection fee, if any, and must be
guaranteed by the consignor in writing.
SECTION EIGHT. All COD shipments will be subject to an additional notification charge of $10.00.

DETENTION OF VEHICLES WITH POWER UNITS-ITEM 350
When attempting to perform a pickup or a delivery and the vehicle is detained through no fault of the
carrier, a charge of $25.00 per quarter hour or fraction shall apply after the expiration of free time, subject
to a $100.00 per hour minimum. These charges are in addition to all other charges. The following free time
shall apply:
Under 2000 lbs.
15 minutes
2001 to 5000 lbs.
30 minutes
5001 to 10000 lbs.
60 minutes
10,001 – 20,000.
90 minutes
Over 20,000 lbs.
120 minutes

DOCUMENT PICKUP CHARGE
When requested, carrier may subject to availability, pickup documents for import cargo at Brokers, Freight
Forwarders or other facilities. The charge for this service will be $40.43 per shipment, subject to customers
fuel surcharge
Item 450
EXTRA LABOR-ITEM 450
When requested by the consignor or consignee, extra labor will be furnished by the carrier for loading or
unloading. At each location where extra labor is used, the charge therefore will be $30.00 per man per hour
or fraction thereof subject to a minimum charge of $240.00.
Time shall be computed from the time the extra labor arrives at the place of pickup or delivery until loading
or unloading is completed. This charge will be in addition to all other charges and will be accessed against
the party requesting the extra labor unless other instructions are listed on the Bill of Lading.
