Conditions of Program Participation
1.
Student meets program eligibility requirements.
2.
Student/parent/guardian has followed procedures established by high school/college for
enrollment.
3.
Student/parent/guardian agree to assume any financial responsibility incurred by the district if a
student withdraws or fails a course.
4.
Student/parent/guardian agree to assume responsibility for transportation, and if eligible, to apply
for reimbursement through the established procedures.
5.
Student/parent/guardian agree to the credit/subject/grade information as indicated on the reverse
side.
6.
Student/parent/guardian understand that credit earned at one institution may not transfer to
another.
7.
Completion of a course required for graduation and receipt of official college transcript must
take place prior to high school graduation date in order for student to participate in graduation
ceremony.
8.
Student/parent/guardian understand that withdrawal from any CCP course does not insure re-
enrollment in a similar course at the high school.
