This worksheet is intended to assist in tracking an employee’s Unscheduled Absences. PMHCC policy 601:
Attendance, Punctuality and Paid Leave Time,
Item 5 states: “Employees who are absent from work for three (3) or more occurrences in a rolling six (6) month
period, shall be deemed as excessively absent.”
Item 6 states: “An occurrence is defined as a non pre-approved/unscheduled absence of one (1) or more
consecutive days at a time. If an employee is excessively absent, he/she will be subject to disciplinary action up
to and including termination.”
