About this form
We want you to receive all the help you are entitled to. This form may seem long but it is designed to help you give
us all the information we need to pay you the right amount of financial assistance.
You must fill in all parts of the form unless there is a note telling you to go to the next part of the form.
Filling in the form
Use black ink to fill in the form. Do not use pencil. If you make a mistake, just cross it out and put the right answer
next to it. Do not use correction fluid or tape.
Answer ‘Yes’ or ‘No’ questions by putting a tick in the relevant box. If you are picking an answer from a list of
answers, put a tick in the relevant box. Do not put a cross in any boxes. If you answer a question with a cross, we
will have to send the form back and this will delay your claim.
If someone else fills in the form for you, they must fill in the declaration Part 8.
