PILRAP APPLICATION CHECKLIST
(to be submitted with your application)
Please read and initial each item to ensure completion of your application and to avoid processing delays.
All Applicants
1. I have filled out all sections of my PILRAP application.

2. I have filled out the “Educational Debt” section of my application and

understand that attaching loan statements does not replace the
requirement to fill out this section.
3. I have attached a statement for each of my student loans that are currently in

repayment or will be on January 1, 2013. The attached statements include my
balance, interest rate, repayment plan and monthly payment amount for each loan.
For any loans that are in the consolidation process, I have attached the most recent
statements available from my lender(s) prior to consolidation if I have not yet
received updated loan statements. I will submit the updated loan statements once they
are available.
4. I have attached an Employment Verification form signed by my employer.

1. I have attached a signed copy of my 2011 federal income tax return. I

understand that my tax return must be signed on the signature line
even if it was originally filed electronically.
