Class Preparation Checklist
This checklist is provided for student use to help you insure that you do not skip any required steps in preparing for the Fire
Academy. It is not necessary to turn this list in.
Application & Registration
Complete application, sign, and turn in to Gini McDonald
Complete Liability Waiver, have notarized, and turn in to Gini McDonald
Pay tuition & compete registration with Gini McDonald
Medical Requirements
Complete the top part of the Firefighter Medical Examination Certification form and sign
See your personal physician for an examination. Ensure that the physician sees the
Firefighter Essential Job Tasks
Have the physician complete the Medical Certification portion of the form
Return to Gini McDonald
Have a urine drug screen done at a medical lab or physician office of your choice
Have results reported to Gini McDonald in the Fire Academy office.
Uniforms
Carefully review the uniform instructions purchase all uniform elements
See included uniform information sheet
Order and pick up name tag from Lonestar Trophy and Athletics
Have patches sewn on – see uniform information sheet for locations.
Wear first day of class and all subsequent days
Personal Protective Clothing (PPE)
Rent or obtain PPE
If department issued PPE, have fire chief complete PPE certification form

Meningitis Vaccine Requirement

Submit required documentation within the time frame noted on the web site
