of this packet.

Waiver of Liability Form

Complete the form as indicated. This form must be notarized. Do not sign until you are at the Notary

Public. Sign the form before the Notary and they will complete the certification section. Submit this

form with your application for admission.

Firefighter Trainee Medical Examination Certification

Complete the top portion of the form. See the physician of your choice and have your physician

complete the “Physician Certification” portion of the form. Be sure the “Firefighter Essential Job

Tasks” are included with the form when you provide it to the physician. This form must be dated no

more than 90 days prior to the start of your academy.

You can register without submitting the drug screen and medical examination. However, these must

be received no later than the first day of class.
