FOLLOW INSTRUCTIONS BELOW IN PREPARING THE TAX RETURN
1.
List all shares of stock you hold of any corporation, including mutual funds and money markets, formed under
the laws of the United States, any state, or any foreign Government. List in Schedule A: number shares; whether
common or preferred; dates acquired and complete name of stock.
2.
List all bonds, notes or any other form of certificate or evidence of indebtedness, issued by any corporation,
(profit or non-profit), association, company or limited partnership formed under the laws of this State or of the
United States, or of any other State or Government, whether interest is or is not paid. List in Schedule B: Date
acquired, face value, exact and complete description as to maturity date, rate of interest, etc.
3.
List all public loans or bonds you hold issued by any State, any foreign country, or any county, city, borough,
township, or school district, except those issued by the United States or the State of Pennsylvania or any of its
sub-divisions. List in Schedule B: date acquired, face value, exact and complete description as to maturity date,
rate of interest, etc.
4.
List all mortgages you hold, irrespective of the location of the property or whether or not interest is paid thereon.
DO NOT INCLUDE MORTGAGE ON YOUR HOME, OR MORTGAGE WHEREIN YOU ARE MORTGAGOR.
List in Schedule C: date acquired, address of mortgaged property, balance due January 1.
5.
List all judgments held by the person, firm or corporation making this return. List in Schedule C: date acquired,
name of debtor, balance due January 1.
6.
List all promissory notes or bonds given by individuals, irrespective of the residence of the makers or whether
interest is paid. List in Schedule C: date acquired, name of debtor, balance due January 1.
7.
Articles of Agreement bearing interest: List the amount due you as of January 1 on any agreement and/or
contract which bears interest.
8.
Accounts bearing interest: List the amount due you as of January 1 on any and all accounts which bear
interest. (Do not include accounts in banks, banking institutions or savings accounts or bank money market
accounts).
9.
Equitable interest: Any assets from a trust located in another state list on Line 9. On a separate sheet, please
list the assets of the trust, your present age, and whether you are sole or partial beneficiary.
10.
All monies loaned or invested in other states, territories, the District of Columbia, or foreign countries.
11.
General-Partnership: any co-partnership, located in Philadelphia, owning or holding personal property of the
classes set forth in instruction 1 through 10 must file a Personal Property Tax return.
12.
Total Assessment for county purposes. Total of lines 1 through 11.
