Brand Specific Report for the First Quarter 2005

If there is a cigarette or tobacco tax increase, you will receive a letter from the department
in the mail. Currently all information pertaining to the Directory of Approved Brands and the
Minimum Price List, is only sent out by e-mail, to all subscribers of the cigarette and tobacco
e-list (contact Dawn Johnson if you are not currently on the e-list). The statute for cigarette and
tobacco taxes increased to three years – starting July 1, 2004. All cigarette and tobacco tax forms
need to be retained for three years, unless it pertains to the Brand Specific Reports, which have a
five year retention requirement.
The Department of Revenue is enclosing a copy of the first quarter 2005 brand specific report for
your convenience. There is now also a fill-in version of this form on the Department's website
The version that you can fill-in will be in acrobat reader. You open the
form and then you can type in the information on your computer. The pages will total for you
automatically. You must save this on your computer, and then print one copy to mail in to the
Department (keep a copy for yourself). There is not a way available yet to electronically file this
form.
If you use an excel file or computer generated report, you may still send those in attached to a
copy of the brand specific report. You must still fill in the distributor information at the top
of the page. Please send only the original report, no extra copies are requested at this time.
For TOBACCO ONLY DISTRIBUTORS, you must complete this form each quarter, too.
We need to know if you are purchasing any roll your own (RYO) products directly from the
manufacturers, which is then sold in Iowa. If you are not buying RYO product, just fill in the
permit number and the distributor information at the top of the page, check the NONE box, sign
and date the form. ALL IOWA Cigarette or Tobacco Distributors, whether there is Iowa
purchases or sales must file this report. Failure to timely file this report each quarter will
subject you to a penalty of $200.00 or greater.
