KUSF Address Information
The following clarifies the purpose of each address that is required on the KUSF worksheet:
Section 1- Line 1
Provides the complete mailing address of a carrier’s corporate headquarters.
Section 5- Line 24
Provides the complete mailing address of the KUSF contact person. This address will be used for
future KUSF mailings, including statement/ bill, unless NECA KUSF Administration is otherwise
notified by submission of the form below.

The address listed in Section 5- Line 24 (Contact Mailing Address) will be used for future KUSF mailings and
statement/ bill unless the following form is completed:
