2000/2001 Remittance Worksheet Instructions
Additional KUSF Information and Filing Considerations
$
Carriers should be aware that penalties of one percent (1%) per month (12% APR)
cumulatively will be automatically applied to filings received after the 15th day of the current
month.
$
The KUSF instructions provide descriptions of revenues and the worksheet lines to be used.
Correct completion of the Section 2 information will avoid edit errors, revisions, additional
contacts and possible penalties.
$
Incomplete forms will be returned to carriers for completion. These will be returned and
may result in late filing penalties.
$
Forms without officer signatures will be considered incomplete forms and will be returned
for signatures and may result in late filing penalties.
$
Carriers requiring subsequent changes and/or corrections to information initially filed on
KUSF worksheet(s) should file a revised worksheet. Revisions should be filed for errors
such as filing incorrect data on a worksheet or arithmetical errors.
$
Annual true-ups are to be used if the carrier has filed estimated data during the year that is to
be trued-up to the actual data at the end of the funding period.
