Section 3- Remittance Calculation
Line 16- 2000/2001 KUSF Contribution Rate
The 2000 KUSF Contribution Rate of .0492 for wireline telecommunication carriers and
.0466 for wireless carriers is calculated for the revenue data months of March 2000 through
February 2001.
Line 17- Gross KUSF Assessment
Multiply the amount on Line 15 by Line 16 as indicated on the worksheet and enter the result
on Line 17. Incumbent LECs collecting KUSF on a per line basis should report the actual line
counts (all multi-line, single-line business, and residential) for each month.
Line 18- KUSF Support Payable to Eligible Telecommunications Carriers
Enter the monthly amount of KUSF support due to the carrier, if any, from the KUSF Support
Calculation Worksheet provided by the administrator.
Line 18a- Lifeline Discount
Enter the monthly number of lines eligible for Lifeline assistance. In accordance with the KCC
Order, dated December 27, 1996, the Lifeline discount per line is $3.50, effective March 1,
1999. Multiply the number of lines by the discount per line and enter the result on Line 18b.
Line 19- Net KUSF Remittance
Subtract Lines 18 and 18a from Line 17. If the result is positive, enter the Net Remittance to
be paid. If result is negative, this indicates that a KUSF payment is due to
the carrier and NECA will credit the carrier’s account in this amount. Refunds will only
be made for significant credit balances or KUSF payments due to the carrier.
Section 4- Change in Company Status
Line 20 - Business Status Change
If business status has changed in Kansas, enter the date that the business was sold, merged, or
discontinued. If there is a carrier name change, enter new name and the
effective date.
Line 20a- Survivor Company Name
If business has been sold or merged, provide the survivor company name. If the business has
