ANNEX A – INCIDENT REPORTING
If any of the following situations occur, you are to notify the chain of command (normally your
OC) within the following timeframes. If the OC or suitable alternate cannot be conducted you
are Authorized to contact COS-AAFC [0417 952 671] or DCDR-AAFC [0419 781 282] direct.
Incident

Fatality or serious injury;
IMMEDIATELY
Sexual harassment/abuse;
Incidents that may attract adverse public notice.
Child abuse/neglect;
Within 2 hours
Motor vehicle accident.
Lost Cadets
Within 2 hours of being late.

Suspension of any staff or cadet’s service;

Defence OH&S Incident Reporting
In the event of an incident, accident or injury to any member, DETCDR must ensure that Form
AC563 Defence OH&S Incident Report is completed the following authorities are notified in
accordance with the timeframes stated on Form AC563. The Occupational Health Safety and
Compensation Branch (OHSC) and COMCARE contact numbers can be found on the cover
page of the form:

Form AC563 Defence OH&S Incident Report is available from the Administration section of
the AAFC Resource Centre. The form must be faxed to the National Safety Advisor on 02
6266 1058 and also their Wing Safety Advisor.
