9. The midwife agrees to maintain professional liability insurance acceptable to the
College of Midwives of Ontario (“CMO”) at all times while working pursuant to the
project. Evidence of such liability insurance must be obtained by the CMO prior to the
midwife practising pursuant to the project.
Remuneration
10. The midwife will be paid by:
a. Institution xxx (paid by hospital, health district)
b. Individual xxx (paid by physician)
c. Agency xxx (e.g. TPA via MPG)
d. Discreet program contract/funding)
[this will change depending on whether it’s an employment agreement or a “partnership”; if the
midwife is being paid by the Transfer Payment Agency then that should be stated just to make it
clear that the other party to the agreement is not responsible for payment.]
Quality Assurance Requirements
11. The midwife agrees that she is required to continue to meet her quality assurance
program requirements pursuant to the CMO Quality Assurance regulation, which
includes continuing education requirements and providing each client with a client
evaluation form for completion after the client’s care has been completed.
Evaluation of Project
12. The parties agree that the project will be evaluated at X and X months and that a report
on the evaluation of the project will be provided to the CMO for its review and
feedback. Key questions that will be considered by the parties in the evaluation process
and are included in the report include:
