Federal Form 1040, Page 1 and Page 2, or federal Form 1041, 1065, 1120, or 1120S, Page 1; and
Form 4835, Schedule F or Schedule C. A Corporation must also submit the same information as would be reported
on Schedule C or Schedule F even though it is not required to fi le those schedules for federal tax purposes.
