APPLICATION & INSTRUCTIONS FOR
ASBESTOS WORKER’S LICENSE

Asbestos Worker is any person who is engaged in asbestos abatement.
Please read and follow these instructions carefully. A correctly completed application will expedite the issuance
of an asbestos worker’s license.
1.
Complete Identification application for D.C. license (BRA-4). (attached)
2.
The applicant must provide the following: (a) a completed application and (b) a copy of a current valid
worker license or school certification that has certification requirements equal to or greater than those of
the District of Columbia. (Delaware, Maryland, Pennsylvania, Virginia and West Virginia are some of the
states that meet the certification requirements).
3.
Report to the Tax Registration Counter, first floor room 1110 for verification of Business Registration.
The business owner shall submit a list of all asbestos workers who will be employed by his/her company
to establish tax liability. Note: Any asbestos worker who arrives without a cover letter indicating employ-
ment by their respective company will be required to apply for his/her own tax certificate which shall
imply that he/she is an independent contractor.
4.
Bring the completed application and supporting documents to the Business Center Room #1100, for
processing. The counter is open between the hours of 8:30 am and 4:15 P.M., Monday through Friday.
5.
If the applicant is self-employed and does not live in the District of Columbia, a Certified Resident Agent
Appointment Form must be completed, notarized and submitted along with the application for licensure.
6.
Applicant must be at least eighteen (18) years of age.
7.
A copy of a driver license or non-drivers identification license is required.
8.
A Clean Hands Form must be completed by all applicants. (attached)
