identity theft. (Provide identifying information for the deceased person, including
full legal name, date of birth, location of birth, Social Security number and
previous five addresses). (Describe your relationship to the deceased person,
such as spouse or executor/trustee of estate).
(Describe how you became aware of the identity theft, such as by receiving
collection notices, bills or credit cards; reviewing credit reports; being notified by
law enforcement or a government agency; or by receiving information from a
family member, friend or attorney who has direct knowledge of the crime).
(Describe in detail any information you have regarding how the identity theft
occurred, including the alleged identity thief, dates and location of the identity
theft and the fraudulent acts themselves, such as any fraudulently opened
accounts, bad checks, credit card abuse, or misuse of the deceased person’s
name, Social Security number, state driver’s license or identification card before
or after the time of the deceased person’s death).
Please begin an immediate investigation of these fraudulent activities and notify
me of the results. Please also provide letters of clearance when your
investigation is complete.
