Registration Form for Previously Appointed Members
This form is for advanced training of all ARB members during their terms. The training must be completed annually by ARB members who
have served all or part of one year. Please complete and return with the $35 registration fee by February 10, 2012. Registration is $25 if com-
pleted by January 16, 2012. There is no fee for registration if you are not a member of an ARB. Seating will be limited. If we do not receive
your registration by the deadline, you may not be able to attend your selected seminar. There will be no onsite registrations.
