This form is for comprehensive training of all new ARB members. New ARB members are required to complete this training prior to serving
on an ARB. New ARB members will be permitted to take the advanced ARB training; however, it may not be substituted for this compre-
hensive training. Please complete and return with $35 registration fee by February 10, 2012. Registration is $25 if completed by January 16,
2012. There is no fee for registration if you are not a member of an ARB. Seating will be limited. If we do not receive your registration by the
deadline, you may not be able to attend your selected seminar. There will be no onsite registrations.
