REAL ID DRIVER LICENSE/PERMIT/NON-DRIVER IDENTIFICATION CARD
PROOFS OF IDENTITY, US CITIZENSHIP OR LAWFUL PERMANENT RESIDENCY
OR TEMPORARY LAWFUL STATUS, AND STATE OF RESIDENCY
GENERAL REQUIREMENTS-
The U.S. Department of Homeland Security regulations establish minimum qualifications standards for the issuance of driver licenses or non-driver identification
cards that federal agencies will accept for official purposes

in accordance with the REAL ID Act of 2005. These regulations require applicants to provide proof of identity; date of birth; Social

Security Number or ineligibility for a Social Security Number; U.S. citizenship
or lawful permanent residency
or temporary lawful status
; and proof of NYS residence. You will be required to
have a new photo taken when you apply for your first REAL ID document. DMV is required to take a new photo at every other renewal or when temporary lawful status is updated.

2 If you are a citizen or lawful permanent resident, proof of U.S. citizenship and lawful permanent residency is only required for REAL ID original issuance transactions.
3 REAL ID documents for applicants with temporary lawful status will expire no later than the expiration on the temporary lawful status proof documentation. REAL ID documents for those with temporary lawful status will require
current DHS documentation for all subsequent transactions.
1. Proof of Social Security Number (SSN) OR Ineligibility for SSN:
♦ You must show your original Social Security Card. If you are not eligible to have a Social Security Number, provide a letter from the Social Security Administration that was issued
within 30 days of your application to DMV and states that you are not eligible to have an SSN. You must also show the Department of Homeland Security documentation that was
used by the Social Security Administration to determine that you are not eligible.
♦ If you have a valid NYS driver license or a non-driver photo ID card that was issued after 12/10/2002 and you are eligible for an SSN, you can show one of the following documents
instead of your Social Security Card:
Your federal or NYS income tax form (W-2) with your full SSN,
OR
Your federal or NYS income tax earnings statement (form 1098 or 1099) with your full SSN,
OR
A U.S. computer-printed pay stub with your full SSN and your name and address.
2. Proof of Identity:
♦ You must show acceptable types of documents that total at least 6 points. If you must show your Social Security Card (2 points), you will need at least 4 additional points. Refer to
the table below for acceptable types of documents and their point value.
♦ The combination of proofs of identity must prove your full legal name. Your full name - first, middle (if applicable) and last - will appear on your document.
♦ You must show one of the documents listed YES under “Date of Birth Proof?” in the table below to prove your date of birth.
3. Proof of United States Citizenship, Lawful Permanent Residency or Temporary Lawful Status:
You must show one of the documents listed YES under “U.S. Citizenship Proof?” or “Lawful Permanent Residence Proof?” or “Temporary Lawful Status?” in the table below.
4. Proof of NYS Residence:
You must show two of the documents listed YES under “Residence Proof?” in the table below.
♦ Each document must show your residence address in New York. An address with a P.O. box is not acceptable as proof of residence.
♦ You may use proof of residence documents that are in your spouse’s name if:
you and your spouse have the same last name,
OR
y
ou show additional documents that prove your relationship (for example, a marriage certificate).
NOTE: If you have a valid New York State Enhanced Driver License document (NYS EDL), a NYS EDL document that is renewable, or a New York State REAL ID Driver License,
you can use that document to meet the proof of identity, Social Security Number, citizenship and residency requirements. If you have temporary lawful status, you may be required to
show new temporary lawful status proof.
The table below lists the documents that are acceptable to prove your name, date of birth, U.S. citizenship, residency, lawful permanent status, and temporary lawful status.
♦ You must provide original documents or documents that are certified by the agency that issued them.
♦ DMV will not accept more than one document of the same type of proof. For example, DMV will accept only one major credit card or one utility bill.
♦ DMV will not accept documents with any alterations or erasures. DMV will confiscate any document that appears to be fraudulent. If you submit a fraudulent
document, you could be subject to criminal prosecution.
♦ First time applications for a REAL ID driver license or non-driver ID cannot be made by mail.
IMPORTANT: Making a false statement in an application or in any proof or statement in conjunction with it, or deceiving or substituting, or causing another person to deceive or substitute in
connection with such application, is punishable as a Class A misdemeanor pursuant to Section 210.45 of the Penal Law for non-driver card applicants, and is a misdemeanor under Section 392
of the Vehicle and Traffic Law for driver license applicants, and may result in the revocation or suspension of your license.
