Semi-monthly or monthly services:
Calculate, deposit and file all payroll taxes and tax returns electronically
Prepare signature ready tax returns and mail to client
Prepare summary reports for 403(b), wage garnishments, etc
Quarterly services:
Prepare Federal Forms 941 and schedule B (if necessary)
Prepare state and local tax returns
Semi-annual services:
Prepare and file worker’s compensation report
Annual services:
Prepare, file and reconcile employee W-2’s
Prepare employer federal, state and local payroll returns
Fees:
Direct deposit fee of $1.75/pay check will be deducted directly by QuickBooks from client’s bank
account after each payroll is processed
$25 will be invoiced by “Service Provider” quarterly for services provided
$100 setup surcharge fee for any church that wants to start the payroll processing at any other
time other than at the beginning of the calendar year.
This Agreement shall be effective on the date hereof and continue unless terminated by either Party
upon notice in writing.
