Reminder:

Both parents must sign this authorization as well as the visa application form in the presence of the Consular

officer.

If applying by mail or through a third party, BOTH parents must sign the authorization and the visa application form, notarize

their signatures on both documents and submit the complete application to the Consulate.

Copy of the birth certificate (long form) must also be submitted.
