One recent passport-sized photograph, no more than 6 months old for each

applicant.

These should be the head and shoulders only against a plain background.

Print the name of the person on the back of each photograph.

For those undertaking the medical treatment



Letter from the treating doctor in Australia with details of the medical treatment plan including relevant

dates, procedure(s) to be undertaken, associated cost and treating doctor’s opinion on the impact of



the treatment on access to medical treatment by an Australian citizen or Permanent resident.



Applicants under the age of 18 – Necessary Requirements

Any child under the age of 18 years who is traveling without either one or both of the parents, a



statutory declaration from the non-traveling parent/s must be provided or

Form 1229 – Completed by both parents.
