I have filed a 2014 U.S. federal income tax return (1040, 1040A, or 1040EZ). However, I have subsequently filed an amended 2014

federal income tax return (1040X). I understand that my student’s application was selected for a review in a process called

“Verification” and that I must provide CSU, Chico Financial Aid & Scholarship Office with my 2014 tax filing information. I

understand that I must complete this form and attach all required documents indicated below.

Note:

Consistent with the Supreme Court decision on the Defense of Marriage Act (DOMA), same-sex married couples must report

both their information if they were LEGALLY married in a state or other jurisdiction (foreign country) that permits same-sex

marriage. Additionally, unmarried same-sex LEGAL parents that are living together must report both their information as well.

Allow 2 weeks after electronically filing your returns or 8 weeks for paper filing.
