Scholarship Program Instruction Form

Scholarships categories are the same as regular memberships. The same rules and policy requirements apply.

Membership categories include:



Adult (age 19 to 61)



Household I (3 or 4)



Household II (5 or more)



Senior (age 62+)



Youth (age 5 to 18)

Mail the completed information, which includes the signed Scholarship Agreement Form,

the Scholarship Application and copies of income verification to:

Or, you may drop the sealed envelope off at the Welcome Center at the same address.

Your application will be reviewed and processed in approximately 3-4 weeks. You will be notified of your scholarship

status by mail. If you are approved for a scholarship, the mailing will include an Award Letter, a blank Membership

Application and a copy of your Scholarship Application, stamped as “approved”.

Upon receipt of an award letter from The Salvation Army Kroc Center, please bring your Confirmation Letter, your

completed Membership Form, and copy of the approved Scholarship Application to the Welcome Center. (Note that

the household income section will be blacked out to protect your privacy.) You must have all three documents with

you to register for membership. Please read the Membership Application carefully and complete it accurately.

When you come in, a Member Services representative will enter your membership information into the computer, set

up your payment structure, take your picture, and provide you with your Members’ Handbook.

10. If you are not approved for a scholarship, you will receive a letter of explanation.
