Section VII: Participant Medical Emergency Card
For Youth and High School teams, an important part of the Medical Emergency Plan is to collect a
Participant Medical Emergency Card from each player. Keep this data on hand at every amateur sports
activity, as part of your Medical Emergency Plan binder. Have each participant’s parent or guardian complete
the form prior to the start of the season.
