WAIVER OF PARTICIPATION
I do not wish to participate in the plan at this time. I understand that I will not be eligible to participate again until the plan's next
entry date. (Please Complete Items I and V).

PAYROLL INVESTMENT ELECTION
I would like to contribute _____% or $_____ of my pay, every pay period, to the plan not to exceed the lesser of 100% of
pay or $13,000 ($16,000 if I am age 50 or older anytime during the calendar year) in the year 2006.
To change future payroll investment amounts or percentages, a new Contribution Election Form must be completed and provided

I understand that adjutements may be necessary to insure the Plan's compliance with the Internal Revenue Code.
