The documents below must be submitted IN THE ORDER OF THIS LIST. ALL

documents must be scanned in .pdf format. NO PHOTOS are to be in the packet. Packets

must be scanned to the AKO folder in TWO (2) scans; one for all medical and physical

documents and one for the rest of the packet. DO NOT SUBMIT ANYTHING THAT IS

NOT LISTED HERE.

___ 2. DA FORM 6285. No more or less than 3; 2 CPTs and 1 MAJ, minimum; Only pages 1, 15

and 16 included. Statement from each board member REQUIRED. Statement may be on pg 16 or

separate memo. Must address potential and capabilities.

___ 3. DA Form 61. AC males choose 2 Combat Arms, 2 Combat Support, 2 Combat Service

Support. 1 Combat Arms must be in top 3 choices. 4 choices at applicants discretion. AC Females

choose 1 Combat Arms, 2 Combat Support, 2 Combat Service Support. 5 choices at applicant’s

discretion. Females may not choose AR or IN. RC Soldiers will only select 3 branches, one choice

must be in the unit vacancy for which applying. Part II must be endorsed by CO and BN

commanders or 2 immediate commanders.
