OCS APPLICATION CHECKLIST

___ 12. ERB/ORB (AC ONLY) or DA Form 2-1 (RC only)

___ 13. DA Form 705. AC must have 2 record APFTs within 12 months of application. Most

recent must be within 60 days of application to HRC. RC must have 1 record APFT within 12

months. APFT must be within 60 days of application to HRC.

___ 14. DA Form 5500/5501, if applicable

___ 15. DD Form 214. For each period of active duty served.

___ 16. DD Form 785, if applicable.

___ 17. CONSCIENTIOUS OBJECTOR AFFIDAVIT. If applicable.

___ 18. DA FORM 2 A or C. (RC only)

___ 19. DA 4187 for any exceptions requested. Must be requested BEFORE packet is submitted.

Contact OCS program office for sample 4187.

___ 20. SIFT test (score over 40) or equivalent if Soldier is applying for AV

___ 21. JPAS printout or installation security manager’s verification of clearance

___ 22. Verification of citizenship memo for Soldiers NOT BORN IN US.

___23. Board approval memo, One commission source memo, commander’s clearance memo

___ 24. DA Form 3474 OR DA Form 3575 (RC only)

___25. DA Form 2-1 (RC only)

SEPARATE SCAN: The below documents must include all lab work and tests associated with the

physical. Both DD Forms must indicate OCS or COMMISSION physical. Tests must be on official

forms from hospital where tests were taken. If applying for AV both documents must indicate

flight physical was taken. DD Form 2808 must have AEROMEDICAL stamp from Ft Rucker.
