information is located in box Ib of the Employer's Quarterly Contribution and Wage Report, form NUCS-

4072. (EXAMPLE:

if you are scheduling a quarterly report with payment for the fIrst quarter of 2000, your

date would be 20001.) Your entry will be repeated back to you. To accept the entry, press the # button; to re-

enter press the * button.

After entering the year and quarter, you will be prompted to enter the Total Wages Paid this Quarter. You

will fmd this information located in box 3 of the Employer's Quarterly Contribution and Wage Report, form

NUCS-4072. The system will not accept decimal points or commas. (EXAMPLE:

if your Total Wages Paid

this Quarter are $163,161.33, you would enter 16316133.) Your entry will be repeated back to you. To accept

the entry, press the # button; to re-enter press the * button.

You will next be prompted to enter the Less Wages in Excess Amount, located in box 4 of the Employer's

Quarterly Contribution and Wage Report, form NUCS-4072. The system will not accept decimal points or

commas. (EXAMPLE:

if your Less Wages in Excess are $4,568.21, you would enter 456821.) Your entry

will be repeated back to you. To accept the entry, press the # button; to re-enter press the * button.

You will next be prompted to enter the amount of Taxable Wages Paid this Quarter, located in box 5 of the

Employer's Quarterly Contribution and Wage Report, form NUCS-4072. The system will not accept decimal

points or commas. (EXAMPLE: if your Taxable Wages Paid This Quarter are $158,593.12 you would enter

15859312.) Your entry will be repeated back to you. To accept the entry, press the # button; to re-enter press

the * button. PLEASE NOTE: If Total Wages Paid this Quarter minus the Less Wages in Excess Amount

do not equal the Taxable Wages Paid this Quarter, the system will not accept the entry .

You will next be prompted to enter the Total Number of Workers Listed on this report for the calendar

quarter, located in box 17 of the Employer's Quarterly Contribution and Wage Report, form NUCS-4072. The

system will not accept commas. (EXAMPLE: if your Total Number of Workers Listed are 3,472, you would

enter 3472. ) You will then be prompted to enter the Number ofWorkers for each month of the quarter, located

in box 18 of the Employer's Quarterly Contribution and Wage Report, form NUCS-4072. (EXAMPLE: month

one had 3365 workers, month two had 3395, and month three 3472.) Your entries will be repeated back to

you. To accept the entries, press the # button; to re-enter press the * button.

Next, you will be prompted to enter the Total Payment Amount in dollars and cents. You will fmd this

information located in box 12 of the Employer's Quarterly Contribution and Wage Report, form NUCS-4072.

The system will not accept decimal points. (EXAMPLE: if your payment amount to be scheduled is $3,515.97 ,

you would enter 351597.) Your entry will be repeated back to you. To accept the entry, press the # button;

to re-enter press the * button.

After entering the total payment amount, you will be prompted to enter the date you would like your

Unemployment Insurance Account credited in MMDDYYYY

format. (EXAMPLE:

if you want your

payment credited to your account on March 31,2000, you wouldenter03312000.) Your entry will be repeated

back to you. To accept the entry, press the # button; to re-enter press the * button.

If you accepted the entry , you will hear that your payment has been accepted and a conflmlation number will

be given. IMPORT ANT -please take note of the confirmation number you are given. You will need this

number to make an inquiry, change, or cancel this transaction.
