Repair/Return Policy and Additional Information
We are happy to provide expedited repair services when needed to meet deadlines for your
upcoming events. Please indicate this information on the requested return date on page 1.
However, please understand that additional fees may be incurred for expedited shipping, if
needed to meet your requested deadline. All effort will be made to keep costs as low as
possible. We send all repair quotes and update information via email; if you have not
received an email from us, please make sure to check for our emails in your spam/junk
email folders.
 
 
All Repairs, EXCEPT those covered under warranty (as seen in our terms and conditions
on ) will be charged a $25.00 Testing and Diagnostic Fee. Once item is
determined to need repair work, this $25.00 will be applied toward the repair charges
incurred. Chase Vision, LLC will perform any and all necessary repairs that WILL NOT
EXCEED $50.00, unless otherwise requested by customer (as noted on RMA form). Should
the repair charges exceed $50.00, the customer will be notified via email for approval.
Most repair charges range from $25 -$50. Every attempt is made to keep repair costs to a
minimum.
 
 
Please make sure to include your credit card information to avoid any unnecessary delays
in the repair/return process. Repairs NOT under warranty sent with RMA forms without
credit card information will not be worked on until payment information is received. We
need your CVV (3 digits on back or 4 on front for AMEX).
 
 
To insure proper handling of warranty merchandise, please include your original
order/invoice number. If you do not have this information, please include an approximate
date of purchase, if known.

If you are shipping more than one item, please list all contents of your package, so that we
can insure proper handling of your merchandise
