The application form on the opposite page must be thoroughly completed and submitted with the

following items: (Completed applications are processed beginning February 15

)

• $200.00 (per child) non-refundable Registration Fee.

• After a student has been accepted, parents will be asked to pay one month’s tuition to hold

a space for each student. (

Information on the FACTS Management Automatic Tuition Payment Program

will be sent after a child has been accepted.)

• Copies of Report Cards, Test Scores, Teacher/Educational Evaluations, etc.

• Copies of any assessments relating to specific needs

• Original Birth Certificate

• Original Baptismal Certificate (if applicable)

• Letter from Catholic parish where you are currently registered (unless a registered member

of St. Thomas More Church, records will be checked so no letter is required)

• New Student Emergency/Health Form

• Health Assessment/Immunization Form (*before the start of school)

• Teacher/Parent Assessment Forms

3 YR OLD PROGRAM - One Teacher Assessment Form and One Parent Assessment Form

PRE-K - One Teacher Assessment Form and One Parent Assessment Form

KINDERGARTEN - One Teacher Assessment Form

1ST GRADE - One Teacher Recommendation Form from current teacher

2ND – 8TH GRADE - Two Teacher Recommendation Forms from current teachers
