I am applying to register my child in St. Thomas More Catholic School for the 2016-17

academic year. I am subject to the following terms and conditions:

• A student registration fee of $200.00 per child MUST accompany this application. After

your child has been accepted, you will be required to send in one month’s tuition payment in

order to secure a space.

• Your first monthly FACTS tuition payment will be deducted from your account in June. You

will be sent information on the FACTS Management Automatic Tuition Payment Program

upon acceptance.

• Refund Policy as stated in the Parent and Student Handbook;

1. If tuition has been pre-paid for either semester or the full year, and if the student

withdraws from St. Thomas More, pre-paid tuition minus the current month will be

refunded. If the withdrawal is before the start of the school year, any tuition greater

than two months will be refunded.

2. If tuition is being paid via FACTS on a monthly basis, there will be no refund. If a

transfer out of the area is required before the start of the school year, any tuition

greater than two monthly payments will be refunded.

Students entering K- 8

grades will be required to participate in an assessment. This

assessment provides us with current performance and learning style information so that we

can provide the best possible instruction for your student(s). It is the goal of St. Thomas

More Catholic School to insure an equal education to all students through differentiating

our instruction based on student needs.

In the event that a learning style difference has been identified through a psycho-

educational evaluation by a licensed psychologist, the school will make reasonable

accommodations for this student. If applicable, this student will be assisted with

appropriate instructional methods. If a student cannot be accommodated by our school

every effort will be made to help with placement in another school.

USE OF IMAGES CONSENT

There will be occasions when images of your child will be used on the school website and/or outside

publications (i.e. brochures, flyers, advertisements, etc.) Please check the appropriate line below.
