Woodbury.

Do not be modest. All of the information will be used by a faculty council to assist with the fair

consideration of your candidacy during the selection process. Failure to complete all portions

of the form could jeopardize your selection. Completion of this form does not guarantee

selection. In the National Honor Society, there are four Criteria areas you must meet: 1

Academic, which you have met; 2

is Leadership; 3

is Service; and 4

is Character. If you

Leadership

List all of elected or appointed leadership positions held in school, community, or work

activities. List only those positions you have held in which you were responsible for directing or

motivation others. Example: student body officer, class or club officer, team captain, church,

etc.
