SICKLE CELL TRAIT (SCT) TESTING PROCEDURE

1. If you have been previously tested for SCT AND you have a written copy of the lab report,

skip to STEP 3 below. (Most states require testing at birth, check with your hospital or

pediatrician to see if they have a SCT test result for you.)

2. If you have not been previously tested for SCT OR you do not have a written copy of the lab

report from a previous sickle cell test, you may do one of the following:

□ Have your personal physician order and draw blood for a SCT screening test. When you have

obtained the written lab result, skip to STEP 3 below.

--OR --

□ Obtain an order for the SCT screening test through the NCAA partnership with Medivo &

Quest Diagnostics. Go to the NEXT PAGE for Medivo procedures.

3. Submit Sickle Cell Trait Testing Results DIRECTLY to:
