To participate in the Norwegian Seventeenth of May Celebration Parade in Ballard, complete this Hold Harmless Agreement and

accompanying Parade Entry Application for your entry and mail it to us. If your entry is a motorized vehicle, whether or not the vehicle

is licensed for road use, please provide with your application either a photocopy of the Washington State Proof of Vehicular Insurance

document or a photocopy of a certificate of insurance covering the vehicle. We cannot accept your application without this

documentation. Please include INDIVIDUAL HOLD HARMLESS AGREEMENTS for each participant with your PARADE ENTRY

APPLICATION.

PLEASE PHOTOCOPY THIS FORM FOR EACH PERSON IN YOUR ENTRY.

Absolutely NO THROWING CANDY. Parade officials are authorized to remove you from the parade for this action.

INDIVIDUAL HOLD HARMELSS AGREEMENT

I/we assume all risk of bodily injury, property damage and personal injury that I/we may incur in participating in the Norwegian Seventeenth of May Celebration

Parade and I/we, for myself/ourselves, my/our child/children, my/our heirs, executors, and administrators do hereby forever waive and release any and all claims

against and agree to indemnify and hold harmless the Norwegian Seventeenth of May Celebration Parade Committee, the Norwegian Seventeenth of May

Committee, the Ballard Chamber of Commerce, Ballard Merchants Association, the City of Seattle, and Seafair and all their respective officers, employees, agents,

representatives, successors, or assigns of any kind from any and all claims which may be made for any cause whatsoever arising as a result of or in connection with

my/our or my/our child’s/children’s participation in the Norwegian Seventeenth of May Celebration Parade.

If you are unable to mail the INDIVIDUAL HOLD HARMLESS AGREEMENTS in advance, bring the INDIVIDUAL HOLD HARMLESS AGREEMENTS with you on parade day

and present them to a parade official. No one in your entry will be allowed to participate without these forms being submitted prior to the start of the parade.

NOTE: THE NORWEGIAN SEVENTEENTH OF MAY COMMITTEE reserves the right to reject or cancel any entry at any time should it be determined the entry detracts from the basic purpose of the

Norwegian Seventeenth of May Celebration Parade and/or the viewing audience. Receipt of this form DOES NOT imply and/or guarantee acceptance into the Norwegian Seventeenth of May

Celebration Parade.
