Independent Contractor Agreement

This Agreement is entered into by and between _________________ (Contractor) and,

Drake University (Client). Client hereby engages the services of Contractor as an

independent contractor upon the following terms and conditions.

Article I – Services Provided

1.1

States generally the type of services to be performed and the applicable fee

structure for such services. As services are rendered, Consultant may facilitate

payment pursuant to the terms of this Agreement by submitting a Consultant Report

to Client in a format acceptable to client. Contractor shall provide services to client

as outlined below:

Project Fees

Project fee shall be a flat fee. Expense reimbursement is allowed only if all criterial

are met on the Independent Contractor Travel Expense Reimbursement

Checklist.

Article II – Term of Agreement

2.1

The term of this Agreement shall be only for such period of time as required for

Contractor to perform the services described in Article I or until Client notifies

Contractor that this Agreement has ended.

Article III – Terms of Payment

3.1

Contractor shall be paid on a per project basis pursuant to the fee schedule set forth

in Article I. To facilitate payment Contractor shall submit an invoice for services.

Contractor must submit a tax identification number to Client as a condition of

payment.
