JOB DESCRIPTION: FORM CARPENTER
Minimum Age: 18 Years
Full time position. Responsible to Foreman or Superintendent. Compensation range: $12.00 - $24.00 /
hour.
General Job Description: Constructs, erects, installs and repairs fixtures, forms, scaffolds, pipes, false
work and mechanical devices. Places pre-fabricated concrete, pours and finishes concrete. Strips forms.
Works with laborers. Works with rigging, loads and unloads materials, directs equipment operators and
truck drivers and generally performs duties as required to satisfy the overall purpose of the job.
Key Areas of Responsibility: Safety, quality and productivity, with minimal supervision, related to the
work. Use of all safety and personal protective equipment for the purpose intended. Cost effective
communication and work scheduling. Cleaning and maintenance of all related tools and equipment.
Hazardous material handling.
Specific Authority: As given by the Foreman or Superintendent, to direct the work of the crew during
supervisory absence. Authority to refuse receipt of materials that are out of specification.
Required Skills / Training: Working knowledge and demonstrated skill with regard to: simple math, reading and writing
English. Reading blueprints and specifications, work layout and planning. All tools and equipment used to perform the work
including all wooden and steel forming systems. Proper hazardous material handling. Proper tie-off procedures. Hoisting
signals
General Description of Tools: All manner of power actuated saws, drills, planes, sanders, cutters and knives; various hand
tools. Compressors, generators, power washers, welders and other large powered equipment.
Tool Policy: Generally speaking, the employee supplies all hand tools belts and bags, and the company supplies all power tools.
Essential Job Requirements: Employees must be able to function well with regard to the following:
HEAVY WORK
 Uncontrolled environmental conditions: Inclement weather, noise, dust, dirt and mud, mobile
equipment, other contractors, and walking surfaces strewn with materials, power cords and tools
is typical.
 Ladders, scaffolding, swing platforms and non-uniform walking surfaces.
 Work sites that change as a function of work availability.
 Occasional lifting up to 75 lbs., routine lifting up to 50 lbs., up to 20 times per hour.
 Reaching at or above shoulder height with wood and metal materials and tools 1 to 10 hours per
day.
 Any combination of standing, bending, climbing, balancing, kneeling, crouching, reaching,
handling, fingering, feeling and walking 8 to 10 hours per day. Climbing ladders, scaffolding,
and structures up to 100 feet high.
 Ability to work different shifts, up to12 hour shifts and overtime as required by the work.
 Ability to work with relative comfort in confined spaces and underground.
 Full use of all limbs and dexterous use of hands.
 Simultaneous mental and muscular coordination, visual and audio awareness of surrounding area.
