INSTRUCTIONS – IMPORTER REPORT
FILING INFORMATION
REQUIRED SCHEDULES:
OVERVIEW:
• PROVIDE ALL INFORMATION REQUESTED AND ATTACH ALL REQUIRED SCHEDULES
• Your report must be postmarked by the LAST DAY of the month following the report month.
SCHEDULE 1A - Tax - Paid Receipts
SCHEDULE 2 -
Tax - Unpaid Receipts
• A separate schedule is required to be filed for each product type and for each seller/purchaser.
SCHEDULE 5X - On-Highway Exempt Motor Fuel Disbursements
• You may be assessed penalties and interest for any late filed return and/or late payment of taxes.
SCHEDULE 11 -
Diversions into West Virginia
• You must file this report even if no fuel activity occurred during the report month.
SCHEDULE 11 A - Diversions from West Virginia to another State
IMPORTER INFORMATION:
Enter your Federal Employment Identification Number (FEIN), Name, Mailing Address, City, State, and Zip Code, and enter your Phone Number, Fax Number,
and E-Mail Address, if applicable.
TAX CALCULATION:
For each product type, enter the number of invoiced gallons in which the tax has not been paid (tax-unpaid) during the report period. Dyed motor fuel, propane and aviation

For each product type subject to the flat rate, enter number of invoiced gallons diverted into WV.
Line 2.
NOTE – You must file an Importer Schedule of Diversions (Schedule 11) for gallons diverted into West Virginia.
Line 3. For each product type subject to the flat rate, enter the number of invoiced gallons diverted from WV to another state.
NOTE – You must file an Importer Schedule of Diversions (Schedule 11A) for gallons diverted from West Virginia.
Line 4. For each product type subject to the variable rate, enter the number of invoiced gallons diverted into WV.
NOTE – You must file an Importer Schedule of Diversions (Schedule 11) for gallons diverted into West Virginia.
Line 5. For each product type subject to the variable rate, enter the number of invoiced gallons diverted from WV to another state.
NOTE – You must file an Importer Schedule of Diversions (Schedule 11A) for gallons diverted from West Virginia.
Line 6. Add Line 1 and Line 2 and subtract Line 3, enter total for each product type.
Line 7. Add Line 1 and Line 4 and subtract Line 5, enter total for each product type.
Line 8. Multiply Line 6 by .2050 and enter total for each product type.
Multiply Line 7 by .11 and enter total for each product type.
Line 9.
Line 10. Enter the number of invoiced gallons of exempt motor fuel sold for taxable purpose (on-highway). NOTE – You must file an Importer Schedule of on-highway Exempt Fuel
Disbursements (Schedule 5X) for exempt motor fuel sold for taxable purpose (on-highway).
Total Invoiced Gallons on Schedule 5X must match Line 10 on this report.
Line 11. Multiply Line 10 by .2050 and enter total for each product type.
Line 12. Add Lines 8, 9 and 11, and enter total for each product type.
Line 13. Add Line 12 for all columns and enter Grand Total Tax Due.
Line 14. Multiply Line 13 by 0.001 and enter total. NOTE – This administrative discount only applies if the report is timely. This discount cannot exceed $5,000.
Line 15. Subtract Line 14 from Line 13 and enter Gross Amount Due.
Line 16. Enter any credit. Indicate reason for credit, example: previous month, or from exporter return, enter the period and if a combined credit, enter amount for each.
Line 17. If Line 15 is greater than Line 16, enter Total Amount Due, (Line 15 minus Line 16). Mark appropriate remittance method, ACH Credit, ACH Debit, or Check Attached.
Line 18. If Line 16 is greater than Line 15, enter the Total Refund or Credit Due, (Line 16 minus Line 15). Mark appropriate box to indicate REFUND requested or CREDIT
to be applied to future return.
CERTIFICATION: PROVIDE THE REQUESTED INFORMATION FOR THE AUTHORIZED REPRESENTATIVE.
