10Column 2 – Special Fuel Products. Special fuel products include product types
160 Diesel Fuel, 170 Biodiesel Fuel, and products classified as special fuel as
listed in the FTA product code list at the end of the instructions.
Column 3 – Other Products. Other products include EOO Ethanol, 130 Jet
Fuel, 288 Diesel Fuel – Dye added, and products classified as “other” motor fuel
as listed in the FTA product code list at the end of these instructions.
The report must be signed by the owner or authorized agent.
Enter the title of the person signing the report.
Enter the date the report is signed.
Enter the phone number.
Print the name of the person signing the report.
Enter the E-mail address and FAX number of the person signing the report.
Instructions for completing Form RPD-41307A, Rack Operator
Schedule of Disbursements
Complete a separate schedule for each product type sold or exported, and for each
schedule type.
At the top of each schedule, enter the following information:
Enter the name of the rack operator.
Enter the FEIN.
Enter the Terminal Code.
Enter the Schedule Type (see schedule types below).
Enter the report month and year that the fuel sales occurred.
Check the appropriate box indicating the type of fuel product that was sold. If the
type of fuel is not listed on the form, check the “other” box and enter the FTA
product code listed at the end of these instructions. Complete a separate
schedule for each type of fuel sold.
