2.
PURPOSE
The purpose of this Memorandum of Understanding is to establish a formal relationship
between the
AAA Hospital and Health
Service, Queensland Health (QH) and the
Eg.
Queensland Police Service, AAA Police Station
for the immunization of e.g.
these QPS
staff
for influenza.
(see Schedules of Services attached)
All participating Parties recognise that cooperation will enhance the successful
implementation of the Staff Vaccination program and each party commits all necessary
resources and best efforts to comply with the terms of this Memorandum.
3.
RESPECTIVE FUNCTIONS AND ROLES
3.1
AAA Hospital and Health
Service:
i.
To identify
AAA Hospital and Health Service
facilities able to participate in
this immunization program
ii.
Provide the vaccines to participating sites
iii.
Liaise with
Eg. Queensland Police Service, AAA Police Station
for the
advertising of participating clinics and times to
Eg. Queensland Police
Service, AAA Police Station
staff
iv.
Provide
Eg. Queensland Police Service, AAA Police Station
with
vaccination records on completion of the program.
v.
Forward a final invoice for the program to
Eg. Queensland Police Service,
AAA Police Station
on completion of the
201x
flu vaccination season.
3.2
Eg. Queensland Police Service, AAA Police Station

Liaise with
AAA Hospital and Health Service
to determine a means
whereby
Eg. Queensland Police Service, AAA Police Station
can
increase its staff immunization uptake
ii.
Notify local
Eg. Queensland Police Service, AAA Police Station
coordinators where QH facilities are participating in the staff vaccination
program
iii.
Organize consent forms for staff immunization program
iv.
Market immunization program to staff
