(NAME OF TEACHER) meets Washington State’s teacher certification requirements and

licensing criteria to teach in the area (he/she) has been assigned to teach. (He/She)

earned a bachelor’s degree with a focus on (SUBJECT). (NAME OF TEACHER) also has

the following advanced degree(s) and/or certificates(s):

Based on the academic preparation of (NAME OF TEACHER), (he/she) is certified by

Washington to teach (grade level) in (list subject).

(NAME OF TEACHER) is not teaching under a waiver of certification requirements. OR

(NAME OF TEACHER) is teaching under a (conditional or emergency) certificate. We are

following the procedures in WAC 181-79A-231 to ensure (TEACHER NAME) is working

Your child does not receive assistance from a paraprofessional. OR Your child receives
